- Experiencing, or at risk of: frailty; social isolation and/or loneliness; barriers to access community-based resources; frequent trips to primary care or ER.

Referral options:
- Health professional referral
- Community service organization referral
- Self-referral
- Family physician or nurse practitioner referral
Details: Referral is required. Referrals can be made by healthcare professionals, community organizations, or through self-referral. Once a referral is received, a Community Connector will follow up with the client to complete an intake assessment and connect them to appropriate services based on their needs.
