- Counselling: Provides counselling sessions that are rooted in trauma‑informed support for anxiety, depression, grief, emotional regulation, and parenting and family support.
- Community Wellness Worker: Practical, hands-on support to help patients navigate care and strengthen their connection to community services. Guiding patients through health and social service systems. Support forms, applications, and referrals.
- Dietitian Program: One-on-one nutritional counselling and support. Access to a monthly grocery program. Access to a learning garden where participants grow their own produce and prepare healthy meals from their harvest.
- Social Work & Navigation: Provides support for a variety of social supports such as housing and income support, legal and family services referral, community resources and safety planning.
- Medical Services: Primary care and longitudinal care for all ages. Including chronic disease management, prenatal and postpartum care, mental health support, immunizations and screening, medication reviews, women’s health, minor procedures and exercise and physiotherapy support.
- A resident of Surrey-North Delta B.C, OR an existing client of EFry Greater Vancouver Programs and Services across the Lower Mainland.
- Do not have a family doctor in B.C.
- Individuals who are currently experiencing 2 or more of the following: Homelessness / Housing insecurity, Substance Use / Substance Use Disorder, Mental Health condition(s), Domestic Violence and/or have been impacted by the Criminal Justice System.
